Become A Health Educator Senior (US)

Job  Description



The Health Educator Sr. will serve as a single Elevance Health point of contact for these members and help them to understand plan benefits and connect them to other Elevance Health physical and behavioral support resources that may supplement what provider teams offer. 

The team focuses on a member-centric care model to understand the member and caregiver situation, and offer guidance regarding community and digital support resources to fully support when and where help is needed Primary duties may include but are not limited to*: Serves as a subject matter expert.


Qualifications

  • Prior experience in health coaching

Responsibilities

  • Identifies and/or coordinates specific health coaching plan needs to address objectives and goals identified during assessments
  • Implements and/or coordinates coaching and/or care plans by educating members regarding health needs
  • Assists with increasing participants' motivational level by addressing barriers and assessing needs to facilitate health behavior change
  • Conducts behavioral assessments to identify individual member knowledge skills and behavioral needs
  • Receives or places call to members who have failed to enroll with disease management to encourage their enrollment in the correct disease management program
  • Monitors and evaluates the effectiveness of interventions and/or health coaching plans and modifies as needed
  • Directs members to facilities, community agencies, and appropriate provider/network
  • Diffuses situations with members who are resistant to health coaching and/or disease management interventions
  • Provides consultation to department associates regarding methods/approaches to help members recognize and overcome barriers to improving their health
  • Refers members to disease management or catastrophic case management
  • May coordinate with others in the multi-disciplinary clinical team to coordinate a health coaching plan for the member
  • May serve as a mentor for orientation and training of new staff
  • Serves as resource expert to Health Educators and provides guidance about processes and systems, including assisting with complex calls, complaints, and, other issues
