Qualifications
- Current RN and NP licensure in the state of practice
- Current CPR certification
- Must be enrolled in both Medicare & Medicaid
- Minimum 1 year of clinical experience as a Nurse Practitioner in a Hospice or Palliative setting
Responsibilities
- The Nurse Practitioner HRA is responsible for performing Health Risk Assessments and occasional transitional care for patients discharged from hospitals to prevent re-admissions
- Conduct comprehensive in-home health risk assessments to identify all active and chronic disease conditions, as well as determine all physical, mental, and social needs present at the time of the visit
- Educate and counsel patient and family based on assessment results, including recommendations
- Performs follow-up visits in transitional care
